If this is your first order, you will need to provide us with a completed Customer Registration Form (PDF) along with your order. Please follow these three easy steps:

1. Review the Qualification Requirements for Test Purchase
2. Fax, email, or mail a completed Customer Registration Form (PDF)
3. Place your order by using our convenient Order Form (PDF)
